Initial Comments:
Based on the findings of an offsite unannounced follow-up survey completed on December 22, 2022, Shangri-La Home Care, LLC had corrected the deficiencies cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A, Chapter 51. The deficiencies were cited as a result of an unannounced onsite state re-licensure survey completed on 9/21/2022.

An offsite follow-up survey conducted on December 22, 2022 found that Shangri-La Home Care, LLC had corrected the deficiencies cited under 28 PA Code, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. These deficiencies were cited as a result of a re-licensure survey completed on September 21, 2022.
